Launched in 2015, the ASHRM Patient Safety Award recognises innovation that promotes patient safety and overall excellence within an organisation. The award celebrates organisations who have introduced initiatives or programs that promote a culture of safety.

The winning project is their “Human Factors approach to Root Cause Analysis”.
This project was developed to standardize and improve their Root Cause Analysis process. Adventist Health System found that they weren’t getting to heart of the problem, that the investigation wasn’t identifying the true cause of an error. Too often we are focused on simply the ‘what’ of an incident occurring and not the ‘why’. By addressing the human factors associated with an incident, Adventist Health System could establish not just deeper underlying problems across their health network, but arm their risk managers with the tools needed to implement effective and lasting change.
Abington Hospital, this years' winners, demonstrated their use of a structured process and standard language which has drastically improved their ability to proactively identify risk. They created a programme called "Create a Safe Night Program", which helped to address the potential for clinical deterioration in patients at risk of declining during the night. The program used multidisciplinary teams to help create situational awareness and develop enhancement plans for patient care.
The 2016 winners, Greater Baltimore Medical Centre (GBMC) demonstrated how their Lean Daily Management principles helped improve safety and reduce total patient safety incidents.
The Lean Daily Management approach trains employees to identify and eliminate wasted time and resources in their own jobs and encourages staff to work with colleagues to improve processes. After GBMC adopted this approach, instances of catheter associated urinary tract infection fell by 92% and surgical site infections fell by 85%.
